Mayo Clinic Referral

I recently went to Mayo Clinic in Phoenix AZ on a referral for a second opinion for a AAA that was repaired. I am being told that BCBS will reimburse me for my hotel and meals for the time I was there and testing was being performed. How do I place a claim for reimbursement?

I called the number on my card. It sent me to a voice mail in Oklahoma. The message said they would call me back within 24 hours. it was the middle of last week and no response so far. Any other suggestions? Thanks...
